What Are the Differences Between Breast Augmentation and Breast Lift Surgeries?
Breast augmentation and breast lift surgery are both popular breast enhancement procedures, but they address very different concerns. Understanding how each surgery works—and what type of result it creates—can help you make a confident, informed decision about your body. While the purpose of breast augmentation is primarily to add volume and enhance your breast shape with implants, fat grafting, or both, a breast lift reshapes the breast tissue itself, elevating your natural breasts to a more youthful position on your chest. Many people are surprised to learn that these surgeries are not interchangeable and that choosing the right approach depends mainly on your individual anatomy, goals, and the quality of your skin and breast tissue.
Breast Augmentation vs. Breast Lift: What Do They Actually Do?
Breast augmentation uses silicone or saline implants, fat grafting, or a combination of both (composite breast augmentation) to increase your breast size and create fuller, rounder contours. Breast augmentation is typically something patients seek if they feel their breasts are naturally small, have lost volume after pregnancy or breastfeeding, or want more upper-pole fullness. Because implants add volume, this procedure can also enhance cleavage and improve the balance between your breasts and hips.
A breast lift, or mastopexy, works differently. Instead of adding extra volume to your breasts, Dr. Gougoutas reshapes your existing breast tissue, removes excess or stretched skin, and repositions your nipples to a more youthful position. In cases of severe sagging, Dr. Gougoutas may recommend incorporating an absorbable mesh to provide additional internal support. A lift restores firmness and height but does not make your breasts larger. It’s ideal for women experiencing sagging (ptosis) due to aging, weight fluctuations, pregnancy, or genetics. If you like the volume of your breasts when they are repositioned in a bra, but do not like the look of them without a bra, then a breast lift alone may be right for you.
What Are the Pros and Cons of Breast Augmentation With Implants?
Pros of Breast Augmentation With Implants
- Increases volume and fullness to create a more proportional and curvier figure.
- Boosts cleavage and enhances your overall silhouette.
- Improves breast symmetry, making uneven breasts look more balanced and consistently sized and shaped.
- Offers long-lasting results.
Cons of Breast Augmentation With Implants
- Does not resolve sagging. If your breasts droop significantly, implants alone cannot give you a lifted look.
- Breast implants are medical devices that may require replacement or revision in the future.
- Carries potential risks, such as capsular contracture, implant rupture, or rippling.
- Leaves small scars, typically well hidden in the breast fold or the outside edge of the areola.
Breast augmentation is an excellent choice if you are primarily seeking increased size and fullness. However, if your main concern is that your breasts are sagging or drooping, a lift—either alone or combined with implants—will likely be the optimal choice for you.
What is Breast Augmentation With Fat Grafting?
Fat grafting uses liposuction to remove fat from areas of the body where there may be extra (often the abdomen, thighs, or buttocks). The fat that is removed is then injected into the breasts to enhance volume.
What Are the Pros and Cons of a Breast Augmentation With Fat Grafting?
Pros of Fat Grafting
- Uses all your own tissue, so you don’t need to worry about potential short and/or long-term implant-related complications.
- Has the added benefit of sculpting the body from where the fat is removed (abdomen, hips, thighs).
- Can be selectively added to specific areas of the breasts where contour may be compromised (upper pole, cleavage area most commonly).
- Changes with your normal weight fluctuations.
Cons of Fat Grafting
- At least 20% of the transferred volume of fat is broken down by your body, making the operation less predictable than augmentation with implants.
- May take more than one operation to achieve your desired volume.
- Cannot fundamentally change the shape of the breasts.
- In rare instances, it may lead to lumps or bumps within the breasts that may be felt through the skin (fat necrosis).
- In rare instances, it may complicate future mammogram interpretation.
What Is a Composite Breast Augmentation?
A composite breast augmentation combines both implants and fat grafting. “Why is this done?” you may ask. There are certain situations where an implant alone may not achieve a patient’s desired results, such as those hoping to avoid augmentation under the pectoralis muscle (sub-pectoral augmentation) or those needing extra volume to blend the implant into the surrounding tissues in the upper pole.
What Are the Pros and Cons of a Breast Lift?
Pros of a Breast Lift
- Repositions sagging breasts and nipples to a more youthful height.
- Removes and tightens excess stretched skin, improving breast shape and firmness.
- Enhances symmetry and overall breast contour.
- Creates a rejuvenated breast appearance without implants, giving lift without added volume.
Cons of a Breast Lift
- Does not significantly increase breast size; in fact, breasts may look slightly smaller afterward due to the removal of redundant skin.
- Can result in more visible scarring than breast augmentation, though scars typically fade with time.
- The natural aging process will continue, so your breasts may gradually droop again.
A breast lift is best for someone who is happy with their breast size but wants a perkier, more lifted shape and better nipple position.
Why May Dr. Gougoutas Recommend Mesh With My Breast Lift?
In cases where the breasts have experienced an extreme amount of sagging, such as following massive weight loss, an absorbable mesh is used to add strength to the breast tissue that is lifted. Even though the mesh is broken down over a period of three to four months, it leaves behind a thin scar band which suspends the lifted breast tissue- this is often referred to as an “internal bra.”
How Do You Know Which Procedure Is Right for You?
Deciding between a breast lift and breast augmentation depends on your aesthetic goals and how your breasts look. When making your decision, think about the following questions:
- Do you want added size or just a more lifted look?
- Do your nipples point downward or fall below your breast crease?
- Have your breasts lost volume after pregnancy or weight loss?
- Is your primary concern sagging, size, or both?
During a consultation with Dr. Gougoutas, he will evaluate your breast tissue, skin elasticity, and proportions to determine which option will provide the most natural and flattering results, or if you would benefit from a combination of a lift and augmentation.
Can You Combine Breast Augmentation and a Breast Lift?
Absolutely. In fact, many women achieve optimal results by combining the two procedures. A breast lift with implants addresses both major components of breast appearance: volume and position. This combination is especially beneficial if you’ve noticed both deflation and sagging after pregnancy or significant weight changes.
A combined augmentation and lift can:
- Restore fullness and projection
- Eliminate sagging
- Improve nipple position
- Create a rounder, perkier, more youthful breast contour
Combining a lift and augmentation in a single surgery means you also benefit from one recovery period instead of two.
In What Situations Should a Breast Lift and a Breast Augmentation Be Separated?
Though one recovery period is preferable to most patients, there are certain situations where combining a breast lift and a breast augmentation is not recommended:
- In cases of extreme breast sagging resulting from normal genetic variation or from massive weight loss.
- In patients who are unsure if a lift alone will give them the look they want, and may not want to effect too much change at once.
- Patients with existing implants who want to significantly downsize their implants.
The above situations are only general guidelines. At the time of your consultation, Dr. Gougoutas will evaluate your individual anatomy and determine which operation, or sequence of operations, will give you the best chance of achieving your desired results.
