When the volume of the breasts is enhanced, whether through implants, fat grafting, or both, the appearance of the NACs is exaggerated, often making them look unnatural or unfeminine. A periareolar mastopexy places an additional incision around the NAC through which a small amount of extra breast skin is removed. This both flattens and sculpts the NAC while giving the breast a more rounded and pleasing shape (link). Differences Between Gender Affirming Breast Augmentation and Cisgender Breast Augmentation
The surgical techniques used for gender affirming breast augmentation are identical to those used for cisgender breast augmentation. Fat grafting may also sculpt other areas of the body from which fat was removed, though this is highly patient-dependent. The volume and shape achieved with fat grafting are, however, far less predictable than those gained with implants. Over a period of roughly three months, the body will absorb some (~20-30%) of the volume that was gained right after surgery. For this reason, patients may require more than one operation to achieve their goals.
Composite Breast Augmentation (Implants + Fat Grafting)
Composite breast augmentation offers the best of both worlds, delivering predictable volume with implants and enhanced shaping and texturing through selective fat grafting. As composite breast augmentation combines two procedures, recovery may be slightly longer than each procedure alone.

How Is a Gender Affirming Breast Augmentation Performed?
All gender affirming breast augmentations are performed under deep sedation. You will be breathing on your own for the procedure and not require a breathing tube down your throat. You will, however, feel no pain, be aware of the surgery in any way while it is being performed, nor remember anything about the surgery itself.
Breast Augmentation with Implants
Augmentation with implants alone involves making a four to five cm incision within the natural fold under the breasts. A space is then developed under the breast where the implant will ultimately sit. This is called the plane of augmentation. During your consultation, Dr. Gougoutas will discuss in detail the various planes of augmentation and which is most appropriate for you. The implants are then inserted using a special device called a “funnel,” which allows the implant to be inserted without actually touching the implant, thereby reducing infection rates. An implant-only breast augmentation typically takes one hour to perform. Dr. Gougoutas does not perform transaxillary or transumbilical breast augmentation.
Breast Augmentation with Fat Grafting Alone
There are three steps to fat grafting. First, the fat is removed from parts of the body where extra fat tends to accumulate through several small (three to four mm) incisions using liposuction. Common sites for fat removal are the abdomen, flanks, and thighs. Excess fluid is then removed from the fat, resulting in a consistency similar to custard. The fat is then injected into the breast through one to two points of insertion using a blunt needle called a cannula. The fat is not all injected at once, but is done slowly in different areas of the breast over several hundred passes of the cannula. Fat grafting augmentations typically take one hour to perform.
Composite Breast Augmentation (Implants + Fat Grafting)

After surgery, you’ll be monitored and released home. You’ll be wearing a surgical bra or other support garment—it’s important to continue wearing this garment to minimize swelling and support your breasts as they heal. If fat grafting is performed, you will likely have a secondary compression garment.
Depending on how physically demanding your job is, you can return to work within one to two weeks when you feel comfortable. You can resume light activity within the first week. Short walks are encouraged to promote circulation. However, raising your arms, heavy lifting, and strenuous exercise are prohibited until Dr. Gougoutas clears you.

Likewise, if you lose weight, the volume of fat in your breasts may shrink. How much weight must be gained or lost to see these changes depends on the patient.
What are the risks of gender affirming breast augmentation?
